Publish Your Passport Application With All The Current Required Documents
Form DS-11 is only one of the several documents that are needed when you submit your pass port application. This form is required for most requests, particularly if you have never received travel documents. If you have not had one issued in the past 15 years, you should also use this document.
Obtain Form DS-11 online or at a local facility that processes them for your replacement pasport. This document cannot be submitted through the mail because you must sign it in front of an official who must confirm your identity. They will ask for identification to verify you are the person submitting the application.
All of the paperwork is to be given to a passport agency or acceptance facility. Facilities include local post offices, the county clerk of the court or public libraries. They are given the authority to accept applications and to verify you are the person who signed the document. Agencies are located in large cities and are regional offices that can accept the paperwork and process it.
Expediting services can submit your application for you. They require that you give them specific information and have been licensed to accept it and submit the application for you. They are capable, under some circumstances, of completing the process in less than a day. The charges for this service are significantly higher than normal processing. The cost is proportionate to the amount of time left before it is required.
When you submit your documents, you must provide primary evidence of U. S. citizenship. The options that you can choose from include a previously issued passport, a certified birth certificate or a Certificate of Citizenship. Check online for the full list of documents that can be submitted to prove you are a U. S. Citizen. When you receive your completed travel document, your documentation will be included with it or may be mailed under separate cover.
You must submit present identification to prove that you are the person submitting the form. You can include a U. S. Passport that has expired, a driver's license or military ID. Since some forms of ID must be retained by their owner, you can submit a photocopy of both front and back in lieu of the document. The original document, however, must be provided at the time you sign your form.
When primary identification is from another state, another form, secondary identification, must be provided. It must include your personal information including name, date of birth and date of issuance. This must be copied along with the primary identification.
When disabled people need to obtain a travel document, they can assign a caregiver to complete the transaction for them. Specific forms are required and the disabled person's signature must be witnessed. If the caregiver is signing for the disabled person, another form is required. In each case, acceptable identification must be provided by the caregiver and photocopied for inclusion in the packet of information.
